Application Error when generating a Forms Doc

I get the following error when I try and generate a Forms Doc in Forms 4.5.7.1.6 on an NT 4.0 workstation.

I get the following error when I try and generate a Forms Doc in Forms 4.5.7.1.6 on an NT 4.0 workstation.
F45DES32.EXE - Application Error
The instruction at "0x004d4c00" referenced memory at "0x0000006a" The memory
could not be written.
Oracle has not been able to give me a solution or reason for why this is happening. If anyone has some insight into this problem I would appreciate it if you could help me out.
