The 32 Bit version only supports OCX, but the OCX implementation in Forms 4.5 is incomplete.
It seems that you are upgrading from 16-Bit to 32-Bit. Why don't you use the lates version of Forms (Version 5 -> Developer/2000 R2). This version has build-in TABs and a working OCX implementation.
