Conversion of Oracle Forms to Windows based format

Does anybody know of a program or service that can convert Oracle forms created in SQL*Forms 2.3 to a Windows 95/NT format.

We are intending to migrate a database away from Oracle 6 to SQL Server, and the forms need to be exported so that the data can be accessed by a Windows 95/NT PC.
