Designer/2000 screen/menu associations & generator?

Does anyone know of a GOOD explanation of how menu modules are associated with screen modules when generating from Designer/2000? I see how to set preferences when generating menus & forms separately, but it just doesn't make sense. I have looked at the help files for designer, "The Oracle Designer/2000 Handbook" by Anderson/Wendelken and "The Designer/2000 Handbook" by Oracle Press (two books with the same name!!), but still don't get it!

In the books, they always show the menu module at the top of the module structure diagram. But unless I play with the preferences, the menu is not called - the DEFAULT menu is called (whatever that is...).
