ON-LOGON problem in Dev 2K 4.W

I'm re-generating forms from Developer/2000 1.2 to Developer/2000 1.4W. I'm having problems with a simple login screen that takes the place of the Oracle default login screen using the ON-LOGON trigger and LOGON procedure. It worked in 1.2, but in 1.4W when it tries to log on I get:

F45DBG32 caused an invalid page fault in module CA0W32.DLL at 014f:10028011

and I have to close out.

Any suggestions? I'm using Oracle7 Server Release 7.3.2.1.0, Forms [32 Bit] Version 4.5.7.1.6, and Windows 95 (yes, the DLL is on my machine). I haven't run into any other re-generation problems.
