We also had this problem running on an HP, with Forms 4.5. We finally determined that it was a sychronization problem -- when the form went off to do a long running job, and then you
opened another application over the top of the forms window. When you close the overlapping application, it doesn't redraw the Forms 4.5 screens. It will eventually come back, as long as the server is processing correctly.

Check for errors on the server, and it if is just a long running job, the screens will come back -- cancelling can actually compound the problem and cause locking in the db.
