How to get Oracle instance name and forms generation date/time

[Quoted] [Quoted] I am trying to build a string that I will append to the Window Name so that when users screen print a problem we know where, when, by whom and which form.

This string would contain:
(1) the User Id (I know this one),
(2) Oracle Instance name (we have three instances PROD, DEV1, TEST
each addressable by Listener and defined in the TNSNAMES.ORA file),
(3) the form name,
(4) the date/time stamp of this form.

I need help with numbers 2 and 4.
