I have had this error (ora-12500) when the ORACLE_HOME values listed in oratab and listener.ora do not exactly match the real ORACLE_HOME for the instance. We were using symbolic links. We had run out of space on our /oracle mount point, but still wanted the new version of oracle to be under /oracle/product, so we created a symbolic link to point to another mount point and used the link name in oratab and listener.ora.
After we received this error, I replaced the symbolic link reference with the actual location of ORACLE_HOME, and I was able to successfully connect.
