OLE and Forms 4.5 V1.2/1.3

We are attempting to create a form which contains a OLE object. When the OLE object is a database object, the form hangs and/or GPF's in F45rd.dll or F45r.dll. When the OLE object is NOT a database object, the form appears to work just fine, except for the obvious problem that the link is not stored.
Our current environment is Developer/2000 V1.2 or V1.3 for Windows 3.1 and Windows NT or Windows 95. None of the combinations appear to work, however.

Has anybody else tried this successfully and if so, any suggestions?
