This may be way off the mark, but...

I have worked on ORACLE platforms where the version of PL/SQL used by sqlplus isn't the same as the version used by Forms. Therefore, things that work in plus either aren't accepted by Forms (at least you know about it), or sometimes are accepted but don't work right.

Check your versions of PL/SQL in the banners for sqlplus and Forms and ask ORACLE if that could be causing your problem.
