Forms 4.0/NT: GPF in USDMEM.DLL

Hello,

Has anybody encountered the following GPF when running Forms 4.0.13.18 (any 4.0.x version) on Win NT 3.51:

   Module: USDMEM.DLL
   Location: 0001:00EE

The error occur mostly when exiting a form, or "switching" between forms using Open_Form, but no pattern can be determined.

Any ideas? ..
