DO NOT FILL this questionaire. It's a local CGI application, made to look like it's coming from the Oracle Corp. That is not the case and there is no guarantee that your name, address and the company will not be abused. Please, report this message to phishing-report_at_us-cert.gov
At the very least, this is spam. This may also be a mechanism to for harvesting email addresses or phishing. Also, links in the paragraphs for the companies below the questionaire are bogus.
