database vault example

i'm having some trouble following the example given in the database vault admin guide. i was wondering if anybody has encountered the same thing.

i've got oracle 10.0.2.0.3.0 on rhel 5.1 (both 64 bit) and installed the 10.0.2.0.3.0 database vault.

the example begins on page 2-3 of the admin guide. i get error messages in the first step, where the dv_acctmgr role is added to the data dictionary realm. specifically, i get

"A error has ccurred saving the configuration information.
Realm Authorization to Oracle Data Dictionary for Realm DV_ACCTMGR already defined "

It gets added anyway, so i go on and till step 4: secure the employees in hr schema and i get

"A error has ccurred saving the configuration information.
 Realm Object %,SCOTT.% already defined in Realm scott realm "

i hadn't installed the sample schemas so i substituted scott for hr...

again, it gets added anyway...

so next i try to create a user and grant select any table to it as system.

I get ora-00604: error occurred at recursive SQL level 1 ora-47401: Realm violation for grant system privilege on SELECT ANY TABLE
ora-06512: at....

Did i screw up the install or configuration on this thing or is the documentation really just buggy?

Also, does anyone have any opinion on actually using database vault (my boss is asking)?
