I would also investigate the parameters as mentioned, especially the optimizer_index_cost_adj. This parameter's default value of 100 is not beneficial for an OLTP application. We have experienced the CBO choosing to ignore indexes due to this parameter being to high. So, depending on the type of application, ensure the parameters are set appropriately.
