Does the file exist?
Answer: yes, but the weird thing is, when you try to do fuser on the dbf file, apparantly the operating system does not see it.

fuser -cu /u01/oradata/ax5.old/data014.dbf /u01/oradata/ax5.old/data014.dbf: fuser: No such file or directory

Did you read the DBWR trace file?
What does it say?
