As manuals states explicitly it creates it. Beware, if file exists it is overwritten.
If Oracle can't create file it throws an exception. do you used dreaded
"exception
 when others then
 null;
"
?

May also be that there is a bug but if you do not tell us Oracle version and Operating system
we can tell you much more.
