There are several bug repots on metalink that this might match but the posted information does have a close resemblence to bug# 1948533. If you have a support contract with Oracle you can open a Service Request and try to have it confirmed. Being that the bug report was closed as being non-reproducable there is likely no fix.

If upgrading from this old release is not possible then in general stable systems start experiencing crash type problems when resources are exhausted. That is the system has or is using all available memory, cpu, etc...

Try running statspack (See Performance and Tuning manual) reports during peek load times and see how things look. Tuning a few poorly performing statments might relieve the problem if it caused by Oracle resource problems and not OS resource problems. Take a close look at OS resources: memory, cpy, number of processes supported (max value for forked processes), etc....
