Thank you for the detailed and insightful response.

> We have again confirmed that the child table (BORRWERPHONE) has the approapriate FK and PK index. On the parent table, we have also confirmed that the referenced column is indexed (in this case the PK of BORROWER table).
> 'ON DELETE CASCADE' is enabled on the FK constraint for BORROWERPHONE table.
> We have also checked on other tables that may reference BORROWERPHONE table. This table is not a parent table to any other table.

This issue came to light after a patch release on the application (internally developed using .NET front-end). There was no evidence of this issue before.

Any other ideas on what to look for? How can we make these enqueue waits go away?
