I can't say as I see a platform in the OP.

But that won't stop me from speculating.

1. Monitor your pool space. All sorts of weird things can happen, and RMAN uses the large pool. How big is yours?
2. What filesystem are you using? A perusal of 9205 rman bugs on metalink shows some examples of inappropiate configuration. Some jfs filesystem could be boning you. Use appropriate monitoring.
