Anyone successfully applied patch 10.1.0.4 on HP-UX Itanium RAC?

I tried to apply 10.1.0.4 over 10.1.0.2 on HP-UX Itanium, but failed at the CRS patch stage. I couldn't find a lot of information except from what's in /tmp/oifcfg.log with the following:

utopen:12:Not enough space in the backing store
utread:3: problem reading the page errno=9, phy_offset=19000,

errstring = Bad file number
ibctx: problem reading the bootblock
proprinit:problem reading the bootblock or superbloc 26

I opened an iTar with tech support more than 7 days ago, and so far no solution
