How can i recover a 9i RAC?

Our corp will implement a 9i RAC with 3 nodes.
i want to konw about the under 2 points:



1, When the OS of one node is crashed, how I restore it (os and
oracle)?how i re-join it into RAC?



2, If i add the fourth node in the future, what changes should be done
with the primary 3 nodes ?
