Not necessarily.

No. However the farther apart the version, the less like it is to be supported. For example, a 9.2.0.4 client and a 9.2.0.6 database are probably fine. But an 8.1.6 client against a 9.2.0.6 database is definitely a problem.

Each patch comes with a list of fixed bugs, so if you are specifically affected by one of those, then you will have to upgrade.

There is a compatibility matrix posted somewhere on Metalink if you look for it.
