How are you determining the execution plan that was used during execution?

From a 10046 trace file?
From a tkprof of a .trc file, with explain=user/pass ? From an explain plan?
from v$sql_plan?

trust the 10046 trace. read it - the raw file. do not trust an explain plan for ( ... ) do not trust

 tkprof infile outfile explain=user/pass ...
