Oracle Support provided a fix via an iTAR. A bug was filed and an existing script was run that provided a fix. I don't want to recklessly broadcast what the fix was, in the event that it does not apply to your issue. I don't know if this will be made public or not, but if it is going to, we'll probably want to change the table name.  

BUG: 3644544 DIRECT PATH LOAD SQL*LOADER-951: ORA-26028: INDEX UNUSABLE STATE Again, this was 9.2.0.5 on RHEL 3.0 ES (no updates). Secondary non-unique index left in unusable state after sqlldr DIRECT load.
