Just in case the problem would not be as 'oracle-internal' as in the answers you just recieved...

last time i was confronted with it, a remote server had lost its connection just during a request (just don't remember wether it had crashed or if it was due to network-problems...)

far as i cought it, there is somthing about the ALTER SYSTEM KILL SESSION, that wants to inform the respective session that it was killed. So, if there is no way to get through to the session, there is no way (i know) to kill it, untill the process opend goes where the session went (just as you said: 'kill -9').
