I'd install 8.1.7 in a new ORACLE_HOME and then apply the 8.1.7.4 patchset. Then, I'd upgrade the 8.1.6 instance to 8.1.7.4 in one step.

But to be honest with you, I'd go to 9i instead. I'm not sure if that is possible for your situation.
