it seems your are facing a bug... but I have not found any bug relevant to ora-16145.

You should open an iTAR to Oracle support, and eventualy ask for a bug to be created (I don't think so -> 9.0.1).

maybe a supervisor job could be set up right now (to restart stdb)...
