Another alternative not already discussed in this thread is mod_owa: http://www.sharemation.com/~dmcmahon/modowa.htm. I've used this for test configurations and it worked OK, although it sometimes crashes when shutting down Apache. A version for Apache 1.3 and 2.0 is available.

One worry though, you have to store the DAD password in clear text in the Apache configuration :-(.
