As previously mentioned, you cannot invoke Oracle forms in 9i in the way you are trying. You are trying to run a form similar to 6i in client-server mode (ie hit the run button and up pops your form).
In 9i, client-server is not supported (foolishly IMO), you need to run the form via a web browser and use an application server on a middle tier.
