I suspect that "Benefit1", "Benefit2", etc. have actual names. I recommend
you use something approaching the actual title of the benefit for your
column title, i.e., OpticalBenefit, DentalBenefit, etc.
I also STRONGLY recommend you read a book on database design before starting this project. It will save you hundreds of hours (not to mention the education received).
