Were you on HP/UX:
Fix:
1. Install the following O/S patch or newer:
PHNE_26551 1.0 HyperFabric B.11.00.0[0-12] cumulative patch
for HP 11
2. Relink all Oracle components (i.e. 'relink all')
I would suggest opening an iTAR at metalink. Or, at least, downloading any patches that may be appropriate.
