Does Oracle enable Restricted Session mode other than by command?

Other than issuing the 'Alter' command, are there any other unusual circumstances under which Oracle will leave a database in restricted session mode? Such as server crash during cold backup or in the event of media recovery becoming necessary?

Did not see any entries in the alert log while investigating why db was left in restricted mode etc...
