Can you please send the output of the following commands 
1)sysdef|fgrep "SHM


SEM



proc"

2)ipcs
3)ps -ef|wc -l
4)Processes parameter in all Oracle instances highlighting the problematic instance
5)swap -s
6)vmstat 5 5
