Do you not have access to Metalink ?

I typed in 'autonomous transaction fetch out of sequence' in the advanced search section there, and got the bug reference from the first page of results.

The bug is exactly what you see - the COMMIT of the autonomous transaction appears to close the cursor of the loop so you get ORA-01002.
