Crystal Reports/Seagate Analysis/Crystal Decisions or whatever it's called these days. Created by Seagate anyway, and I'm pretty sure one of the versions is a cut down freebie, that can still do everything you list below.

I'm not 100% sure that it's free. It may only be free if you have another one of their products.

I've always found the seagate products easy to use, and at the same time they allow the advanced user to do relativly complex things.
