At the risk of stating the obvious:

"ORA-01654 unable to extend index string.string by string in tablespace string
Cause: Failed to allocate an extent for index segment in tablespace. Action: Use ALTER TABLESPACE ADD DATAFILE statement to add one or more files to the tablespace indicated."

In other words, -um- you need to add more space to tablespace CUST1. Perhaps the index segment CUST1.SYS_C00664 is in the same tablespace as the data segment for CUST1 ... not so good, either!
