How to create documents like Oracle's?

Dear all,

Oracle documents come with both HTML and PDF formats. If we are to create system documents like Oracle's, what are the most handy tools? By the way, it is much better if we can make a document in one format and save as another format; therefore, we only have to make one format while supporting both formats. (even better if Word 97 DOC format is also included) Note that the document here I mean one with the rich text presentation as well as the 'outline' mode, i.e. a hierarchical structure, probably with contents and indexes support as well. Thanks in advance.
