As you have noticed, doing an ALTER SESSION does set the NLS_DATE_FORMAT ---- thus you would see that client-side settings can and do override the server-side settings.

Where are you "seeing" the old format ? If you are using a client program (sqlplus) on Unix, check the Unix environment variables. NLS_DATE_FORMAT can be defined in the shell and it would then override the server-side settings. If you are using WinXXXXX clients, check the Registry entries for the oracle software installation for NLS_DATE_FORMAT.
