Okay, deduced the problem to the oracle password file not being created just before CREATE DATABASE. When that happens, you get the missing oracle password file in the alert log.

If you then create the oracle password after CREATE DATABASE, you then get the ORA 12701 in the alert log.

In short, you HAVE to create the password file BEFORE the database is created.
