Oracle and AIX - loadext

I'm having a problem getting the pw-syscall extension to load automatically from inittab. It appears to load correctly but it is definitely not loaded after a reboot and I login.

Here's the line from /etc/inittab:
loadext:2:wait:/etc/loadext -l /etc/pw-syscall >/tmp/loadext.log 2>&1

It's the last line in the inittab file.

Here's the output from the call (piped to /tmp/loadext.log):

 Oracle Kernel Extension Loader for AIX

       Copyright (c) 1998,1999 Oracle Corporation

 Successfully loaded /etc/pw-syscall with kmid: 0x146cc64  Successfully configured /etc/pw-syscall with kmid: 0x146cc64

Kernel Extension Version: 3

Everything appears fine, but when I login I do a loadext -q /etc/pw-syscall (or try to start the Oracle instance) and I find it is NOT loaded.

Does anyone have any ideas what may be happening?
