I have seen this a lot on NT. In all the cases that I have seen as are described. The problem was that the archiver had not finished doing the archive of the redo. This (as Sybrand points out) can be caused by too few or too small redo. In addition, most people that install Oracle on NT put the redo and data and everything else on one physical drive or set of drives. So the drive set is flooded with IO and the archiver cannot possibly keep up.
So increase the size of your redo logs, have at least 4 and put them on a different spindle. (not a different drive letter on the same physical drive.)
