Not having experience with Forms5 though, but Forms4.5 on a standalone WIN98 with Personal edition 8.0.5, you will have to set up TNS for the Forms application. SQL Plus as delivered with Personal does not need it. See also my previous post on this subject in comp.databases.oracle.misc within the last 2 months.
