The 8.1.5.1 version number seems to be incomplete, but let's assume it is 8.1.5.1.0. In that case you should install that version only as patches are cumulative.
The second question puzzles me a bit. Do you expect Oracle to repeat descriptions for everything they fixed in various releases. They expect you to go from one release to another, so there's no real need to do that.
