Is OAS on the same machine as NT service pack 4. If so, you will need to upgrade to OAS 4.0.8.x. SP4 is not supported (or works)

4.0.6.4 is the initial release of OAS and several new features ie. jservlet, beans etc
have been introduced as well as several bug fixes...
