   Yes, I did run across this error early on in the setup of the OAS. Unfortunately, I don't recall what exactly caused the problem. I do know that I had to go back over the instructions on setting up the web enabled forms in order to get rid of this error.

   Sorry, I can't be of more help. Maybe someone else here has had the same problem?
