Kimball's DBMS Mag article on hierarchical dimensions outlines the basics of setting up hierarchical dimensions using a "link" table that contains b-tree-like information, etc.

Seemed interesting, but I couldnt really figure out how to implement it.

For the simple case of a customer dimension hierarchy mentioned in the article, how exactly would one create/maintain this link table?
