The previous answer to this issue is correct. Our application runs on Oracle 7.3.2 on HPUX 10.20 and we had to up several of the semaphore parameters (in particular the semaphore undo parameter). When using SAM on HPUX, there are a number of pre-configured kernel parameter sets that you can choose, including 1 for "OLTP database system".
