The original poster didn't specify which version does he use. If he uses 8i then he could use SQL*Plus for database creation. In 8i you can do anything from SQL*Plus, since you can connect as internal with SQL*Plus.

As a matter of fact, it is mentioned somewhere in the documentation that server manager will become obsolete with future releases.
