Creating Second Database

Hello!
I've installed an instance of Oracle 8i on an NT 4.0 box and was able to create a database without too much difficulty. However, when I try to create a second instance / database using the Oracle Database Configuration Assistant, I get error ORA-12560, TNS: Protocol Adapter Error. This error occurs after the DCA creates and starts the new instance, and during the act of database creation. I realize that I haven't provided much background info here, but can anyone tell me where I might need to begin looking in order to correct my problem and get my database up and running?
