I have in the past tried to start an instance where there are already too many running - so not enough memory - but the svrmgrl prompt is just returned - there are no error messages. Your ORA-27146 indicates a problem with the OS :
ORA-27146: post/wait initialization failed
Cause: OS system call failed.
Action: Examine errno, and contact Oracle Support.
Are you running the correct release of Oracle for the OS ??
